2nd visit (treatment plan) 45 minutes:    $100.00

Subsequent Visits - 30 minutes:    $60.00

Telephone Consultations - 5-20 minutes:    $15.00 - $40.00


Child Consultations

1st (intake & physical) 60 minutes:    $70.00

2nd visit - 30 minutes:    $45.00

Subsequent Visits - 20 minutes:    $35.00
